Membership Disclaimer
Unused monthly services expire at the end of each billing cycle. They do not roll-over month after month. Memberships require a minimum commitment of three (3) months. Membership fees are billed automatically on a monthly basis to the payment method on-file and renew each month (unless canceled in accordance with the terms below). After the initial commitment period, members may cancel with a minimum of thirty (30) days’ written notice. Cancellation requests received after a billing date will take effect in the following billing cycle. Membership benefits are non-transferable and valid only for the enrolled member. All services are subject to medical eligibility and provider approval. Suite Wellness reserves the right to modify membership benefits, pricing, or terms with prior notice.
